Position Summary:

The Registered Nurse Case Manager is a member of the Medicaid Community Health Team (CHT) The Registered Nurse Case Manager participates in the Spoke program by providing comprehensive and coordinated services for patients undergoing Medication Assisted Treatment Services (MAT) specifically buprenorphine treatment. The primary focus of the Medicaid CHT is to assure that clients receive coordinated care that addresses all the aspects of necessary services.


Scope Of Service:

The Medicaid Community Health Team (CHT) is a multi-disciplinary team designed to coordinate access to high-quality, cost-effective and comprehensive Medication Assisted Treatment Services. The goal is to ensure that clients receive coordinated care that addresses all aspects of necessary services. The Medicaid CHT partners with the Brattleboro Memorial Hospital (the CHT Administrative Entity) and Grace Cottage Family Health and Hospital buprenorphine prescribers.


Qualifications:

Education/ Experience: State of Vermont Registered Nurse license. Minimum of one year psychiatric/substance abuse nursing experience or two years general nursing experience.

Other Requirements: Maintains a cooperative professional working relationship with staff including but not limited to medical staff, and other departments, as well as outside agencies, community members and our patients.

  • Maintains confidentiality of patient information according GC policy and federal/state guidelines and regulations. Able to lead and motivate people; excellent interpersonal skills.
Requirements:

Essential Functions Of The Position:

  • Coordinate and provide access to chronic disease management, including self-management support to individuals and their families.
  • Coordinates and provides access to individual and family supports, including referral to community, social support and recovery services.
  • Coordinates and provides access to long-term care supports and services
  • Provides quality-driven, cost-effective, culturally appropriate, and person and family-centered services.
  • Facilitates the development and implementation of the multidisciplinary plan of care for assigned patients.
  • Develops a person-centered care plan for each individual that coordinates and integrates all of their clinical and non-clinical health-care related needs and services through a Health Home Plan of care.
  • Provides information, support and education to motivate patients toward a desire to change and increase their capacity to care for themselves. Establishes goal setting and action planning for each patient.
  • Problem-solves concerning patients with substance abuse or dual diagnoses using a goal-oriented, systematic process.
  • Demonstrates proficiency in comprehensive patient documentation. Demonstrates a capacity to use health information technology to link services, facilitate communication among team members and between the health team and individual and family caregivers, and provides feedback to practices, as feasible and appropriate.
  • Participates as a Medicaid CHT Member and supports VT Blueprint for Health initiatives. Attends CHT and practice staff meetings as scheduled/required.
  • Documents treatment session outcomes in EMR.
  • Assists with developing and preparing reports and documentation as required by the Vermont Blueprint for Health Grant.
  • Other related duties as designated by supervisor.
