LA NJ - GA KY REMOTE MTM Pharmacist

We are seeking a highly skilled Remote MTM Pharmacist to join our team for a contract position, starting now and extending through October 2024 with potential for further extension. As a Remote MTM Pharmacist, you will play a crucial role in providing direct patient outreach and medication management services to Medicare patients. This position requires strong MTM experience, the ability to work independently, and a mitment to improving patient outes through evidence-based medication management practices.

Location: HOME OFFICE (Bethesda, MD 20814) Remote (LA, NJ, GA, KY preferred)

Responsibilities:
- Conduct medication adherence/management calls, providing direct patient outreach and support.
- Prevent, detect, and resolve medication-related problems, making evidence-based remendations for optimal therapy.
- Perform thorough medication reconciliations for post-discharge patients.
- Assist in the management of high-risk, high-cost patients to ensure optimal care coordination.
- Conduct telephonic prehensive medication reviews (CMRs) for qualified patients.
- Identify and address patient-specific barriers to medication adherence and disease management.
- Provide clinical decision support, education, and consultations to providers as needed.
- Coordinate care between care managers, providers, and pharmacies to ensure appropriate medication management.
- Collaborate with providers, clinical staff, and administrators to optimize medication management workflows and patient experiences.
- Stay updated on formulary changes and remend alternative pharmacotherapy as necessary.
- Follow evidence-based guidelines for managing chronic disease states in collaboration with providers.

Experience:
- Strong background in MTM required.
- Experience in a health plan or pharmacy benefit management (PBM) setting preferred.
- Retail pharmacists with MTM experience will be considered.

Hours:
- Monday to Friday, 9:00 AM to 5:00 PM EST (flexibility required for potential expansion).

Licensure:
- Ideal candidate must hold at least two of the following licenses: LA, NJ, GA, KY (bonus states: UT, TX).

Requirements for Working from Home:
- Private workspace at home with desk and chair, minimizing background noise.
- Minimum internet download speed of 100mbps and upload speed of 10mbps.
- Ethernet connection required; WiFi cannot be used.
