Care Navigation Annual Wellness Nurse - CIN Care Navigation - Part Time - Days - Full-time / Part-time

This position will support Spring Forest Family Physicians.

Primary responsibilities are:

• Gather and review eligible beneficiary's self-reported health information
• Update the eligible beneficiary's medical, social and family history
• Review beneficiary's Health Risk Assessment to identify potential barriers, risk factors, opportunities for education and gaps in recommended preventive services
• Counsel beneficiary on age-appropriate preventive services based on the beneficiary's health status and screening history
• Furnish personalized health advice to the beneficiary and a referral as appropriate, to health education or preventive counseling, services or programs
• Update the EMR to reflect the beneficiary's most current information

• Current RN nursing license in the state of Ohio; prefer licensure in Michigan and Ohio
• CPR certification
• Bachelors or Master degree in nursing from an accredited school
• Critical thinking and clinical competence
• Effective interpersonal and communication skills
• Knowledge of insurance, federal regulations preferred
• Flexible with hours to met the needs of the department
• Computer literacy including inputting and retrieving information from the computer
• Multitasking and organizational skills
• Mobility and ability to move around department with prolonged periods of standing
