Director of Nursing

We are seeking a Director Of Nursing to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.

Responsibilities:

Before agency can accept referrals, the Agency must complete accreditation process through CHAP. Candidate must be willing to work with consultants throughout the accreditation process and will be solely responsible for completing ALL visits, and paperwork as applicable. Once accredited this will be a permanent full time position. Agency will pay a $3,000 bonus upon CHAP accreditation being successfully completed. Agency will pay $75 per start of Care ,$40 per routine nursing visit,$55 per recertification and $35 per discharge and offer a $40 dollar a week prepaid gas card. CHAP requires agency to service 5 patients for 1 certification and have only 3 active patients at the time of survey. 

• Administer nursing care to ill, injured, or disabled patients
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members

Qualifications:

• 2 years minimum Previous home health care experience  ****REQUIRED****
• Familiarity with medical software and equipment
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
