FamilyCare Health Centers is currently recruiting for a Telephone Triage Nurse for the St. Albans, East Location.

Qualifications:

  • Registered Nurse (RN) License
  • One year of experience
  • Strong analytic Skills, problem-solving and critical thinking.
  • Outstanding written and verbal communication, computer skills.

Certificate/License

  • CPR Certification
  • Access to insured vehicle to perform job duties
  • Maintains valid unencumbered drivers license.

Basic Function:

Responsible for receiving and/or making phone calls to patients related to lab and test results, vaccines protocols, health initiatives, prescriptions/samples that require further review, routine, urgent or emergency situations, transition of care review, follow up, and scheduling of appointments as needed. The employee is part of the medical support team.

Duties and Responsibilities:

1. Demonstrate competency and sound decision making in job performance, through accurate documentation for routine/urgent, and emergency and/or crisis support for the patient, required trainings through completion of orientation checklists; MA/Nursing checklist as applicable, phone triage checklist, electronic health record (EHR) documentation, knowledge of policies related to prescriptions, lab and x-ray notifications, patient message protocols, and other policies and training needed for competent job performance and decision making.

2. Assess patients timely and accurately via telephone and prioritize care according to routine, urgent, or emergent situations.

3. Review messages throughout the day, clarify questions regarding patient medications or test results, request test results for the EHR, document request and contact in the chart before forwarding to a provider for review.

4. Respond to phone calls or inquiries related to test results, prescriptions, complex, urgent and emergent calls. Have a working knowledge of company policies related to vaccines protocol.

5. Create case notes related to clinical task notifications/patient interventions in the EHR and forward to the provider for review and directive as appropriate.

6. Review, update, and reconcile allergies, medication lists, and other fields in the EHR as needed.

7. Communicate plan of care or directive to patients and confirm they are agreeable to the plan of care and document in the EHR.
