Company Details: Heart of Ohio Family Health is a non-profit organization that operates the following six Federally Qualified Health Centers located in Franklin County, Ohio: Heart of Ohio Family Health at Capital Park, Heart of Ohio Family Health at Canal Winchester, Heart of Ohio Family Health at Mount Carmel East, Heart of Ohio Family Health at St. Ann’s, Heart of Ohio Family Health - James B. Feibel Center, and Heart of Ohio Family Health at Whitehall-Yearling (a school-based site). Summary: This position supports the Organization in the following manner:
  • Provides counseling services for mental illness and substance use disorder.
  • Completes screening, brief intervention, and referral to treatment
  • Proactive builds a patient panel through patient outreach and engaging primary care and other providers.
  • Maintains medical records

Reports to: Behavioral Health and Social Services Program Manager

Job Duties, these are considered essential to the successful performance of this position:

  • Assist the providers as needed by administering the highest quality patient care in a sensitive and respectful manner
  • Accurately documentation in appropriate medical record
  • Ensure providers’ orders are properly followed
  • Triage patients in office visits, walk-ins and via telephone to complete SBIRT or crisis counseling services.
  • Perform the tasks of an LSW or LCSW
  • Follows all policies, procedures and work rules
  • Other duties as necessary or assigned

Job Qualifications (Experience, Knowledge, Skills and Abilities):

  • Must be Licensed in the State of Ohio to provide mental health counseling services (LSW, LPCC, or equivalent; LISW preferred)
  • Required experience: at least one-year Counseling
  • Required education: Bachelor’s degree. Preferred: Master’s degree.
  • Must have experience in Mental Health counseling. Experience in Substance Use Disorder Counseling is strongly preferred.
  • Experience with integrated behavioral health preferred.
  • Multi-lingual (Spanish, Nepali, or Somali) and/or experience in working with interpreters/diverse patient populations preferred.
  • Ability to successfully guide others to create an efficient, harmonious work environment
  • Supervision certification or supervision experience preferred
  • Knowledge of federal, state and local laws and regulations pertaining to Social Work Licenses
  • EMR/EHR experience required
  • Competency in working sensitively and 1respectfully with people of various cultures and social status
  • Skills to expertly deliver exceptional health care to the public


Facility Environment:
Heart of Ohio Family Health Centers operates in multiple locations, in Columbus, OH. All facilities have a medical office environment with front-desk reception area, separate patient examination rooms, nursing stations, pharmacy stock room, business offices, hallways and private toilet facilities. All facilities are on the main ground floor and ADA compliant.

This position’s primary work area is the clinical portion of the facility; ie, patient examination rooms, hallways, and other clinically assigned areas.

The clinical area is:

  • kept at a normal working temperature
  • sanitized daily
  • maintains standard environment furniture with adjustable chairs
  • maintains standard office equipment; ie, computer, copier, fax machine, etc. at a normal working height
