Heart of Ohio Family Health is a non-profit organization that operates the following six Federally Qualified Health Centers located in Franklin County, Ohio: Heart of Ohio Family Health at Capital Park, Heart of Ohio Family Health at Canal Winchester, Heart of Ohio Family Health at Mount Carmel East, Heart of Ohio Family Health at St. Ann’s, Heart of Ohio Family Health - James B. Feibel Center, and Heart of Ohio Family Health at Whitehall-Yearling (a school-based site). As a nationally recognized quality leader and patient-centered health care operation, or mission is to provide high-quality, holistic, and compassionate care to meet the healthcare needs of everyone in our diverse community, one heart at a time. We provide healthcare services regardless of ability to pay and offer a sliding fee scale based on income and number of dependents.

Summary:

· The physician is in compliance with laws and regulations pertaining to the organization’s administration of healthcare.

· The physician provides medical services to the patients of Community Health Services in acute care and the clinical office setting

· Establishes and maintains a patient practice establishing health plans with diagnosis and performing non-surgical treatment of diseases and injuries of internal organ systems.

Provide care mainly for adults who have a wide range of problems associated with internal organs

Reports to: Chief Medical Officer

Supervises: N/A

Dress Requirement: Business Casual with white provider coat or scrubs in accordance with Heart of Ohio Family Health’s dress code policy

Work Schedule:

Monday through Friday during standard business hours

Times are subject to change due to business necessity

Job Duties, these are considered essential to the successful performance of this position:

üWorks as an integral part of the team providing care in accordance with the organization’s mission as it relates to indigent and charity care

üMaintains strict patient confidentiality at all times

üCompletes patient history and physical

üDocuments all patient care provided according to policy

üFormulates and directs a plan of care for the patient

üRefers to other providers as needed

üPrescribes medication in accordance with Ohio revised code and Ohio Administrative Code

üDirects patient education for health education, maintenance, emotional and physical needs

üProvides evening and weekend coverage for patients on a rotational basis coordinated by the Chief Medical Director as required

üParticipates in Performance Improvement Program

üDemonstrates effective oral and written communication skills

üEstablishes and maintains a professional working relationship with other members of the healthcare team

üMaintains knowledge of and complies with personnel and medical policies and procedures


üAssists other team members


üAdheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPPA, OSHA, OIG guidelines, other state and federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor, suggestion box, any suspected unethical, fraudulent or unlawful acts or practices


üProvides direct service, case management and collaboration with all multidisciplinary team members in the provision of healthcare to the patients


üParticipates in professional development activities to keep current with trends and best practices in healthcare administration


üPerforms duties in accordance with community, professional and legal standards


üProvides culturally sensitive professional care for all patients registered to be seen at Centers


üAdheres to all correct coding requirements set forth by Medicaid and Medicare


üAttends educational workshops


üAdhere to all of the Organization’s policies and procedures, especially the hazardous, health and safety procedures


üPerforms other duties as assigned

Job Qualifications (Experience, Knowledge, Skills and Abilities)

üLicensed to practice medicine in the State of Ohio


üLicensed in customary narcotics and controlled substances prescriptions


üBoard eligible or certified by an applicable nationally recognized board


üFree of all suspension, sanctions, reprimands or discipline by any licensing board, state, local society, or managed care organization


üFree of malpractice action or allegations of malpractice whereby a settlement payment to a plaintiff of an aggregate of more than $100,000


üAbility to participate as a physician in federal care programs; ie, Medicare, Medicaid, etc.


üKnowledge of medical care laws and regulations and standard care guidelines


üAbility to communicate (orally and in writing) in a professional manner


üAbility to work in conjunction with other employees and business associates


üAbility to maintain an established work schedule to ensure dependability, accuracy of work quality, and a harmonious, consistent work environment

Facility Environment:

Heart of Ohio Family Health operates in multiple locations. All facilities are a medical office environment with front-desk reception area, separate patient examination rooms, nursing stations, pharmacy stock room, business offices, hallways and private toilet facilities. All facilities are on the main ground floor and ADA compliant.

This position’s primary work area is patient examination rooms and office.

The patient examination rooms and office area is:

  • kept at a normal working temperature
  • sanitized daily
  • maintains standard office environment furniture with adjustable chairs
  • maintains standard office equipment; i.e., computer, copier, fax machine, etc. at a normal working height
