A Direct Support Professional / Job Coach is responsible to ensure the safety and well-being of the individuals Blue Sky, Inc. provides services to while implementing individual outcomes, documenting progress and response to program services and to provide individualized training and follow-up support services, enhancing employability and job retention.

Direct Support Professional

  • Obtains and demonstrates a working knowledge of individual(s) history, skills and areas of vulnerability, individual outcomes and emergency procedures.
  • Obtains and demonstrates a working knowledge of program policies and procedures.
  • Provides support, supervision and safety to each individual using therapeutic interventions as specified within procedures and directed by the individual’s CSSP Addendum, Self-Management Assessment (SMA), Individual Abuse Prevention Plan (IAPP), Program Abuse Prevent Plan (PAPP), if applicable and the county’s ISP and/or CSP.
  • Maintains and performs proper distribution and documentation of all prescribed medications, treatments and diets as directed by health care professionals.
  • Assists individuals with daily living skills and/or provide personal care as needed.
  • Assists in transporting individuals as directed by Designated Coordinator.
  • Demonstrates and maintains positive and professional working relationships and open communication with staff and various other persons related to the position.
  • Encourages and provides opportunities for individual’s choice and presenting creative options.
  • Abides by 245D, Human Services Licensing Act and Maltreatment of Vulnerable Adults and Minors.
  • Ensures the accurate, legible and objective recording and reporting of all required documentation including but not limited to: Daily log notes/case notes, health progress notes, outcomes and data collection, medication/treatment records, incident and emergency reports and medical referral forms.
  • Assists in facilitating individual’s participation with life events, family members and in their community, including but not limited to: Birthdays, family holidays, telephone calls, sporting events, volunteering, worship services, etc.

Job Coach

  • Provides necessary job shadowing support (on site job training or coaching), to ease transition of worker into community business site.
  • Assists participant in transferring generalized learned skills to a new site making adjustments that are necessary.
  • Assists participant in meeting production demands of the employment position during their training-in period.
  • Provides an open channel of communication with the employer, families, other team members and placement specialist in regard to progress and performance.
  • Adapts to a number of work site environments involving a variety of tasks, duties, employer/employee relationships, new production rates (of different jobs), operations and procedures, etc.
  • Assists the employer in modification/restructuring of jobs for workers with disabilities, when necessary.
  • Arranges scheduled follow-up visits to provide job stabilization support services for employer and worker, as appropriate.
  • Assists job candidate in transition to community employment through developing social skills, personal awareness and understanding of each individual abilities as well as limitations.
  • Conducts oneself as a positive role model for participants in terms of appearance, socialization, conversation and teamwork.

Qualifications:

  • Must be able to read, write and speak the primary language of the individuals Blue Sky, Inc. provides services to. This must include the English language and may include additional languages as needed.
  • Meet licensing requirements of the state and/or the county.
  • High school diploma or GED.
  • Possess a valid driver’s license and good driving record.
  • Possess personal vehicle with the ability to transport at a minimum of three additional individuals.
  • Computer proficiency and technical aptitude with the ability to use Microsoft products, including PowerPoint.
  • Must be able to follow and successfully complete pre-employment screening and background check requirements.

  • Excellent organizational skills and attention to detail.
  • Excellent time management skills with a proven ability to meet deadlines.
  • Ability to act with integrity, professionalism, and confidentiality.
  • Proactive and independent with the ability to take initiative.
