OVERVIEW OF THE ROLE/JOB

The primary responsibility of your job position is to support the DON in fulfilling all nursing administrative duties, including direct, coordinate and lead MDS Systems, Medicare A, Medicare B and all other insurance providers, as well as conduct care coordination for case management following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities. Working with the DON you will divide nursing administrative duties in a way that will ensure the well-being of the elders, compliance with all state and federal guidelines, and the capturing of reimbursement due the community. Incumbent is delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. Will also serve as Education/Training Capacity.

KEY RESPONSIBILITIES:

  • Manages and coordinates care with the rehabilitation team
  • Assess vilagers in all areas needed to complete annual and quarterly MDS forms
  • Write and update Care Plans as needed per assessments and monitor that acute care plans are current
  • Assess elders for need for restraints and obtain all needed consents and physician's orders
  • Coordinate the process of assessing incontinent Villagers for need for bladder or bowel retraining or toileting programs
  • Implement adaptive behavior programs and other interventions consistent with the care plan for each individual villager
  • May direct the restorative nursing program
  • Assess strategies for capturing the highest burden of care delivered to villagers and is responsible for implementation of strategies to ensure quality outcomes and fiscal responsibility for villager assessments
  • Assists with leadership by participating in daily PSS, weekly and monthly triple check
  • Complete pre-admission assessments and participate in the decision making process regarding new admissions
  • Complete Quality Management Reports based on Quality Measures, develop action plans and report on process and outcomes of these action plans
  • Serve on or chair the Medication Review Committee and Pain Review Committee to champion the most effective use of medications with the lowest dosage to treat the villagers conditions.
  • Participate in or assist with the following programs: Restraint, Pain Management, Quality Management, Fall Management
  • Assist in evaluation of the effectiveness of policies and procedures and make suggestions for revision in the interest of more effective services
  • Provide clinical education to nurses of policies and procedures and make suggestions for revision in the interest of more effective services
  • Perform administrative duties such as evaluations, studies and audits as needed
  • Administer and track necessary inoculations of both elders and staff. (TB, Flu, etc.)
  • Helps maintain the physical, social and psychological environment
  • Ability to maintain professional working relationships with all nursing personnel, villagers and families; present professional image to the villager through dress, behavior and speech
  • Participate in new staff general orientation
  • Serve on committees such as infection control, safety, Performance Improvement, Adaptive Behavior and Culture Change. May serve as chair of some of these committees; attends department supervisor and staff meetings
  • Receives direction from DON and completes delegated tasks in his/her absence.
  • Act as Manager on Call per protocol
  • Functions as a Charge or Treatment/Medication Nurse as required, with or without notice
  • Must follow all safety procedures; reports accident/incident hazards; assist with infection control in identifying risks; follows established infection and universal precautions policies and procedures; monitor the use and handling of equipment to ensure safe usage and avoid waste or breakage
  • Maintain confidentiality of all pertinent villager care information as well as staff information; adheres to HIPAA
  • Ability to make independent decisions when circumstances warrant such action
  • Follow agency procedures when purchasing items on behalf of the agency.
  • Follow agency policies and procedures in carrying out duties.
  • Advise supervisor of concerns and problems that arise within the agency.
  • Safeguard all Bluestem Village funds and property, including those individuals in our care.
  • Performs other duties as assigned by the Supervisor, Administrator, and/or designee.
  • Other reasonable duties as may be assigned.

SKILLS AND KNOWLEDGE:

  • Must have knowledge of insurance procedures, contracts, etc., covering business transactions
  • Must have basic computer operation and typing skills for data input; must be able to navigate touch screen computers
  • Knowledgeable of nursing and medical practices and procedures as well as regulations and guidelines that pertain to long term care
