The Ryan White Care Coordinator will provide non-emergent medical transportation rides for consumers through a voucher system or ride share, to medical, dental and vision appointments, pharmacy medication pick-ups, lab work, and other related services. The coordinator will assist with collecting trip information, trip logs, creating PCF for delivery of services, maintaining spreadsheets, and assisting with high call volume. The coordinator will assist with all voucher programs, PCF approval process of payments and process consumer NMAP referral to enroll and determine a payer source for services. The coordinator will act as a representative of Access to Healthcare Network (AHN) while on duty and will relay information about other programs to consumers as well as problem solve on behalf of our consumers to ensure that they receive a continuum of care and support.

Key responsibilities and accountabilities:

  • Gain and maintain a general working knowledge of Ryan White and HOPWA program services.
  • Gain a clear and working knowledge of the HIV/AIDS Drug Assistance Program (ADAP); establish and maintain appropriate working relationships with the persons and agencies involved.
  • Gain clear and working knowledge of granting NMAP (ADAP) dates based on payer of last resource rule (30 days vs full NMAP dates).
  • Learn, and follow, the payer of last resort rule at all times.
  • Screen eligibility applications received from Part A and Part B agencies to verify the application is completed correctly and all required documents are attached for eligibility and ADAP services.
  • Manage all incoming eligible Ryan White referral data entry from Part A and Part B CAREWare systems in a timely manner.
  • For Part A & B Referrals ability to transfer (mirror) incoming referral data into Part A, Part B and Salesforce.
  • Gain clear and working knowledge of social services available throughout our community; establish and maintain appropriate working relationships with persons and agencies involved.
  • Gain/Possess a clear and working knowledge of Salesforce, Mitel and CAREWare.
  • Complete an eligibility screening to ensure consumers are eligible for assistance based on funding source.
  • Gather information from AHN Ryan White consumer.
  • s to determine the payer of last resort for Medical Transportation Services.
  • Refer eligible consumer to AHN Transportation department.
  • Acts as a liaison between consumers to identify other AHN/Community programs of which consumer may be eligible for.
  • Accurately and efficiently record all pertinent consumer information into CAREWare and Salesforce.
  • Entering, transcribing, recording, storing, or maintaining information in written or electronic form.
  • Return all phone calls, emails, or written correspondence within 48 hours of receipt.
  • Process medical transportation referrals for consumers within 48 hours of receipt unless otherwise noted by a provider and in accordance with AHN policies and procedures.
  • Orientate Program members with specific program information.
  • Track program member appointments and compliance with program guidelines
  • Track funding and process payments for ride share.
  • Gain a clear knowledge of PCF approval process.
  • Complete PCF approval weekly to ensure payments are processed timely.
  • Submit all programmatic reports on time, in a professional format and with accurate data.
  • Complete daily input of all services provided to consumers.
  • Be able to multitask and critically think though a variety of situations.
  • Perform other duties as assigned.

QUALIFICATIONS:

  • Minimum Education: High School Diploma
  • Minimum one year computer experience including Microsoft operating systems, particularly Excel, Word, and Outlook.
  • Database input experience and the ability to use a smartphone required.
  • Required to pass a background check through the State of Nevada.
  • Subject to Medicare/ Medicaid fraud background check
  • Required to pass a drug test upon hire and may be subject to random drug testing throughout employment.
  • Bilingual preferred (English/Spanish).
  • Extreme patience.
  • Is consistently at work and on time and arrives to appointments and meetings on time.
  • Be able to manage one's own time and the time of others efficiently.
  • Be able to actively learn.
  • Be able to operate at the pace of business to meet deadlines, plan and prioritize, and remain calm when dealing with difficult customers.

  • Abide by all HIPAA requirements.

  • Be able to stand between 1 - 2 hours per day.
  • Be able to sit in a vehicle or at a computer for up to 6 - 8 hours per day.
  • Must be able to lift, carry up to 30 pounds with a fair consistency.

  • The noise level in the work environment is usually moderate and the employee usually works in a climate-controlled office environment.
