Remote Licensed Practical Nurse - Contractor

We are looking to grow our team of remote Practical Nurses [Licensed Practical Nurse (LPN), Vocational Nurse (VN), Licensed Vocational Nurse (LVN)].

The Role and Responsibilities

We are looking for licensed practical nurses to serve our partnership with a leading national company that supports remote care coordination for Chronic Care Management (CCM), and other clinical and documentation needs for practices across the country.

The LPN will work closely with the PocketRN team and our partners to ensure continuity of care and facilitate cross-functional communication. Their roles and responsibilities will include, but are not limited to:

  • Complete chart audits and reviews and update the patient and/or POA accordingly, using designated software.
  • Quickly build rapport with patients, POAs, and PRN team members virtually in a respectful and professional manner to ensure excellent experiences and outcomes.
  • Complete assigned audits with patients using designated software.
  • Ensure accurate time tracking and documentation of time spent on each episode of care.
  • Identify and communicate any concerning findings to the appropriate and relevant parties to allow for timely follow-up.
  • Successfully complete technical and clinical training as required within specified timeframe.
  • Follow escalation protocols when urgent scenarios occur during calls.
  • Maintain patient privacy and strictly adhere to HIPAA compliance standards in a virtual care environment.
  • Maintain an active and unencumbered nursing license in the state where care is delivered.
  • Must be available to work a minimum of 25 hours per week, with ability to flex to additional hours.
  • Hours of operation for this partner will likely be Monday - Friday between 8:00 am - 7:00 pm for patients in Central and Eastern Time Zone with potential evening and weekend hours.

Qualifications

  • Education: Must be a graduate of an accredited degree program.
  • Experience: Two years nursing experience post graduation.
  • Licensure: Must have an active and valid compact LPN license.
  • Computer Proficiency: Candidates must have basic computer skills to perform remote work, navigate a virtual desktop, and the ability to learn new software systems. Candidates will need to pass a technical assessment during the interview process.
  • Communication: Able to demonstrate effective interpersonal, oral, written, and listening communication skills. Take initiative and communicate regularly with their cohort leads.
  • Language: Able to communicate effectively, in English, over the phone, video conferencing, and in writing.
  • Professionalism: Provides courteous, empathetic, and clear communication to patients even in stressful scenarios. A desire to go above and beyond for patients. Follow through on chosen work schedule and maintain productivity in a remote work environment.
  • Patient-Centered: Demonstrates patient-centered/patient-valued behaviors. Focuses on patient needs, patient rights, confidentiality, and religious and cultural preferences.
  • Adaptable: Able to learn new systems and workflows quickly and easily adapt to nursing in the virtual care start-up environment.
  • Mission and team-driven: Excited about working with a supportive, compassionate, and collaborative team by using innovative technology to create a lasting impact for millions of patients, caregivers, and nurses.
  • Technical Requirements: You will need a quiet, private room, free of distractions in your home for taking HIPAA-sensitive video calls. You’ll need a fast (at least 50 Mbps required) and stable internet connection. For testing your internet speed, visit www.fast.com.
  • Must have a laptop or desktop computer (No Chrome Books)
  • An additional computer monitor is required
