Licensed Vocational Nurse - Crisis Receiving for Behavioral Health (CRBH)

Department: CRISIS Programs

Location: 7th & H

Reports to: Program Manager or Designee

Status: Full-Time, NOC Shift, Mon. - Fri. 11pm - 7:00am (must be available to work the shift)

FLSA: Non-Exempt

Pay Range: $75,900 - $98,670

Position Summary

 

The Licensed Vocational Nurse (LVN) coordinates and administers medical services to Crisis Receiving for Behavioral Health (CRBH) patients, including vitals, assessing, screening, educating, discharging, and treating patient conditions. We welcome newly licensed graduates to apply.

 

Required Qualifications

 

· Current license as Licensed Vocational Nurse with the State of California required.

· Two plus years of clinical experience with a dverse adult population OR an equivalent combination of education and experience preferred.

· New graduates with licenses are encouraged to apply.

  • Fluency in additional languages preferred.

· Current First Aid and CPR Certification.

· Knowledge of community resources, including Medicare, Medi-Cal, Supplemental Security Income (SSI), and the county's General Assistance Program.

· Knowledge of medications, usages, interactions, and effects of substance use drugs.

· Ability to observe, assess, and record symptoms, reactions, and progress; react calmly and effectively in emergencies.

· Educate patients and provide instructions on proper care and treatment.

· Maintain detailed records.

· Exceptional interpersonal skills.

· Excellent customer service experience, including active listening, problem-solving, and calmness in emotional or stressful situations.

· Computer (PC) literate, including electronic health record, word processing, and e-mail, required.

· Communicate clearly and concisely, both verbally and in writing, and demonstrate a high level of listening skills.

· Adherence to infection control procedures, including but not limited to standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.

· Assist and support the Center/Department/Program to meet standards of High Reliability.

· Ability to work holidays, weekends, and alternate shifts as needed to maintain required staffing levels.

· Successful completion of all required organizational and Crisis program training.

  • Must have a valid driver’s license and insurable driving record.

 

Essential Responsibilities

· Work as part of a multi-disciplinary team with patients experiencing homelessness and co-occurring disorders such as mental health, chronic medical conditions, and substance use disorders at a high acuity care facility requiring 24/7 patient support and monitoring.

· Act as on-site medical professional and direct care of patients according to WellSpace Health policy and protocol.

· Work with various people from low-income populations, diverse education, lifestyles, ethnicities, and cultures.

· Engage with community members experiencing homelessness, disabilities, substance use, HIV( AIDS) infection, and/or mental health crisis.

· Perform LVN duties for up to 22 patients. Prepare rooms and room patients. Intake patients by performing an initial assessment, taking medical histories, recording vital signs, taking temperatures, and performing other required nursing duties for patients under the influence of drugs and alcohol or in a mental health crisis.

· Conduct rounds at a minimum of every 2 hours to check on patient status. Document rounds in EHR.

· Encourage patients’ general well-being through health promotion.

· Follow standardized policies and protocols for patient treatments and conditions.

· Administer injections and PRN according to a standing order.

· Keep accurate records for reporting to various funding and inspecting agencies.

· Maintain files for regulatory compliance and inspection.

· Inventory medications and supplies; rotate inventory regularly to minimize expirations.

· Review patient medical charts and prepare documentation.

· Record all patient interactions in the EHR system and ensure the accuracy of the information in the EHR system.

· Assist with daily operations, laundry, food distribution, cleaning, etc.

· Other duties as assigned.

Physical Demands and Work Environment

The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle, or reach. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.

 

The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
