Job Description

The Physician Assistant (PA), & Nurse Practitioner (NP) function as an independent healthcare provider who work collaboratively among a multidisciplinary healthcare team to provide comprehensive care and services to residents in long-term care communities, for both, inpatient and outpatient settings. He/she demonstrates a high degree of clinical expertise in working with residents with acute and chronic illnesses commonly encountered in long-term care including end of life care needs. He/she is responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination for residents. The Physician, PA, and NP demonstrate professionalism working collaboratively with Executive Directors, Administrators, Directors of Nursing, and facility leaders to assure safe, effective, quality resident care.

Essential Job Functions - Clinical Knowledge

  • Demonstrates knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions.
  • Performs history and physical assessments of patients that are accurate, complete, concise, and relevant for clinical problems.
  • Orders appropriate clinical studies and interprets results
  • Develops, implements, evaluates, and alters residents' plan of care as needed.
  • Effectively manages general medical conditions based on knowledge of indications, contraindications, side effects, interactions and adverse reactions of pharmacologic agents and other treatment modalities.
  • Counsels and educates residents and families.
  • Maintains accurate, complete, concise, and timely documentation in the electronic health record within 48 hours (preferred).

Essential Job Functions - Resident Care

  • Provides care that is resident centered, compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease throughout their lifespan.
  • Facilitates consistent, coordinated care and clear communication among the multidisciplinary healthcare team.
  • Respects and acknowledges Resident Rights for allowing residents to make informed decisions regarding their care and services provided to them.
  • Serves as an advocate/liaison for residents and families in navigating the health care system.

Essential Job Functions - Facility Collaboration

  • Communicates effectively with physicians, NP/PA providers, other health providers, and health related agencies.
  • Promotes mutual respect and trust working within the long-term care facility to support Quality Assurance Performance Improvement (QAPI) goals.
  • Works effectively with the multidisciplinary healthcare team.
  • Communicates and collaborates with the Executive Director, Administrator, and DON to address facility issues and concerns in a timely manner.
