Summary Job Description:

Home and Community Based Services (HCBS) and Children and Family Treatment and Support Services (CFTSS) offers opportunities to better meet the behavioral health needs at earlier junctures in a child/youth’s life to prevent the onset or progression of behavioral health conditions. These services are also intended to help prevent the need for more restrictive and higher intensity services for children and youth.


The CFTSS Service Provider is responsible for providing the services of Community Psychiatric Support (CPST), and Psychosocial Rehabilitation (PSR) to qualifying children and families. The HCBS provider is responsible for providing the services of Caregiver Family Supports and Services, Prevocational Services, Supportive Employment Services, and Community Self- Advocacy Training and Support.  Services are provided directly to the child and/or the family and will often take place in home or community settings.


Service Providers will also provide CPST and PSR during a child’s visitation with biological parents/families. Service Providers will utilize the principals and best practices of Visit Coaching to deliver effective services during these family time opportunities.


Principle Responsibilities: 

  • Participation in the development and ongoing monitoring of the child’s Treatment Plan.
  • Provide direct services to child & family on caseload.
  • Services are provided throughout the five boroughs.
  • Documentation of all service provision and contacts.
  • Attendance at all meetings deemed necessary.
  • Coordination of care with other providers and/or collaterals to assist child in obtaining their optimal functioning level within the community setting.
  • Currently the position is remote and if needed will report to the Brooklyn office.
  • Attend all required trainings.

 

Minimum Requirements: 

  • Bachelor’s degree in social work, Psychology or a related field, plus four years of related experience required.
  • Master’s degree in social work, Psychology or a related field preferred, plus one year of related experience required.
  • Excellent writing, communication and word processing skills required.
  • Ability to travel to various communities.
  • Ability to work a flexible schedule including weekends and evenings.
  • Possession of a valid NYS driver’s license is required.

  

This is an Hourly Position, salary will commensurate with experience.
