Description:

Hospice of the Piedmont seeks a Hospice Licensed Practical Nurse (LPN) with PRN status. This position will serve in and around Charlottesville as well as counties East. Under the supervision of a registered nurse, the Hospice LPN will deliver quality patient care and comfort measures, emphasizing physical, spiritual, and emotional needs. They provide care for patients within the Home Hospice Care Program, managing symptoms and ensuring adherence to the Hospice Plan of Care.


Responsibilities/General Duties (Including but not limited to):

  • Deliver nursing services in accordance with Hospice policies and procedures.
  • Provide and document care at bedside utilizing the Electronic Medical Record (EMR) system.
  • Assist physicians or registered nurses in performing specialized procedures.
  • Administer prescribed medications and treatments according to nursing standards.
  • Observe, record, and report patient's condition and response to drugs and treatments to the RN case manager.
  • Perform patient/family services within the Hospice Plan of Care and organizational policies.
  • Report any changes in patient’s needs and condition to the RN case manager or team leader promptly and accurately.
  • Assist in interpreting Hospice services to patients and families.
  • Communicate regularly with the Interdisciplinary Team to ensure effective coordination of patient care.
  • Participate in Quality Assurance (QA) programs as required.
  • Attend meetings and participate in committees as requested.
  • Engage in professional development activities and maintain professional affiliations.
  • Maintain patient confidentiality at all times.
  • Complete other assignments and tasks as requested.
Requirements:

Qualifications:

  • Education & Licensure: High School Graduate or GED. Graduation from an accredited school of practical nursing. Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia
  • Experience: One or more years of relevant nursing experience required. Hospice home care or nursing home experience preferred.
  • Knowledge, Skills, and Abilities: Knowledge of practical nursing principles and skills for patient care and treatment. Familiarity with medications and their effects on patients. Ability to take vital signs accurately. Proficient in managing aspects of terminal care, health team service coordination, and documentation. Basic knowledge of Medicare/Medicaid reimbursement regulations. Ability to establish and maintain effective relationships with patients, families, medical staff, and the public. Strong problem-solving skills and ability to recommend solutions.
  • Technical and Communication: Basic to Intermediate proficiency in word processing software. Familiarity with office equipment and basic computer skills. Excellent oral and written communication skills
  • Mindset: Emotionally mature and compassionate individual willing to support terminally ill patients and their families. Ability to remain calm and effective in emergency situations. Willingness to work beyond scheduled hours when necessary.
