our Specialized Recovery Services
program. The position will cover Erie, Ottawa, and Sandusky counties.
Responsibilities include outreach to clients,
verification of eligibility requirements,
assessment, and care management services for individuals needing
community-based long-term care.

Qualifications:
Registered Nurse or Licensed Social
Worker or a bachelor's degree in counseling, or psychology with 3 years'
experience in home & community-based
services or case management. Reliable
transportation,
insurance, valid driver's
license, the ability to
satisfactorily undergo
an FBI/BCI check and a Motor Vehicle Record check (MVR) on at least an annual basis.
