Job Description:

As a Pharmacist, you’ll be involved with multiple facets of the home infusion process as you interact with insurance companies, staff members, distributors, Physicians/Nurses, and patients to ensure accurate and safe quality service. You will use your experience as part of the Pharmacy Team to resolve problems when unforeseen roadblocks arise. Perform specific distributive and clinical pharmacy activities and related services as they pertain to providing safe and effective home infusion therapies.

Primary Duties:

The following duties are not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

  • Adheres to organization policies and procedures.
  • Coordinates and facilitates new referral/admissions through collaboration with nursing, customer service, and distribution.
  • Receives, reviews, and verifies all new patient orders and documents are the same on the order form.
  • Assists the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapy.
  • Conducts initial and ongoing assessment of specific patient, drug, and disease information to identify real and potential drug-related problems or needs.
  • Assists the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy.
  • Monitors the outcomes from drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data, and other pertinent patient information.
  • Communicates the results of therapeutic drug monitoring activities to the physician, nursing, and other appropriate persons.
  • Programs infusion control devices as indicated.
  • Provides consultation and answers inquiries from patients, families, physicians, and other healthcare professionals regarding drug and their use.
  • Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians, and other healthcare professionals.
  • Participates in the preparation of progress reports for the referring physician on a regular, periodic basis.
  • Supervises, coordinates and/or participates in the preparation of sterile admixtures, dispenses all pharmaceutical and related supplies and equipment in accordance with federal, state, and local laws and standards of practice which govern the profession of pharmacy.
  • Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Maintains pharmacy records in accordance with applicable laws and regulations.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars, and conferences.
  • Participates in on-call rotation if applicable.
  • Performs marketing support functions as requested.
  • Participates in orientation/training programs for new personnel and current staff.
  • Participates in departmental QA activities.
  • Attends designated department meetings as scheduled.
  • Performs other tasks or special projects as requested by management.

Confidentiality and Compliance:

  • Maintain confidentiality of patient and proprietary information at all times.
  • Observe legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary company information.
  • Understand and comply with all HIPPA requirements and regulations
  • Participate in the process for assessing, managing, and reporting medication-related incidents and equipment errors and/or failures. Accurately completes occurrence forms and forwards them to the appropriate governing bodies.
  • For all activities complies with accreditation, legal, regulatory and safety requirements.

Qualifications: (Education, Certification/Licensure and Experience/Skills):

  • A Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited (Council on Pharmaceutical Education) college of Pharmacy.
  • Currently Licensed as a registered pharmacist in the state of employment.
  • Possess a minimum of two years recent experience in an acute-care setting or one-year recent experience in home infusion therapy.
  • Possess experience and strong knowledge base in intravenous admixture services and intravenous therapy.
  • Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses, and other healthcare professionals.
  • Can work effectively with others to provide safe and effective drug therapy.
  • Can communicate, both verbally and written, with patients, clients, management, and other employees.
  • Can problem solve and supervise technical and other support staff.
  • Must be flexible with work schedule.
  • Possess good organizational and time-management skills.
  • Participation and membership in professional societies and organizations is desirable.
  • Dependable in attendance and job performance.

Additional Requirements:

  • Language Skills: Ability to read, analyze, and interpret common scientific and technical journals, financial reports, government regulations and legal documents. The ability to write reports, business correspondence, policy, and procedure manuals. Ability to effectively present information and respond to questions from managers, company staff members, and patients/caregiver.
  • Mathematical Skills: Ability to work with mathematical concepts such as probability and statistical interference and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Cognitive Demands: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret and extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
  • Computer Skills: To perform this job successfully, an individual should have knowledge of Excel, Word, PowerPoint, order processing systems, and CPR+.
  • Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to sit and stand for periods of time, walk, and regularly lift and/or mover up to 25 pounds. The employee is required to communicate by speaking. The employee is required to listen to the needs of the co-workers and /or customers and have the ability to write and communicate information to other co-workers. The employee is frequently required to climb on a small ladder, reach with hands and arms, frequent bending, stooping, and kneeling.   • Work Environment: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The noise level in the work environment ranges from mild/quite to moderate.
