Job Description
JOB DEFINITION:
  • The Clinician functions as a member of the field-based Community Oriented Recovery and Empowerment (CORE) services. The Clinician provides approved CORE services acting as the main contact for Community Psychiatric Support and Treatment (CPST), while also able to provide Psychosocial Support Services (PSR) and Family Support and Training (FST) as needed to individuals out in the community with mental health and/or substance abuse disorders. This role will employ a variety of interventions, evidence- based practice, and person-centered planning techniques as well as assist with the acquisition and maintenance of natural resources and supports in the community to achieve stability. The Clinician will be on a rotating schedule on an as-needed basis to cover any on call issues that arise outside of regular business hours.

DUTIES AND RESPONSIBILITIES:
  • Oversee a manageable caseload of clients that are enrolled in CPST CORE Services as well as provide other CORE services on an as needed basis.
  • Monitors the pharmacological effects of prescribed psychotropic medications and notifies the prescriber of their reported effectiveness.
  • Conducts initial and ongoing assessments of assigned clients to document strengths, needs, goals and resources.
  • Participate in strengths based, person- centered goal planning that incorporates the values and principles of CORE as well as the individual's life areas, such as community connectedness, development of natural resources, physical and mental health wellness, education, employment and self-help, to create an individualized plan of care.
  • Utilize psychotherapeutic interventions, evidence-based practices when available to assist individuals to reach their fullest potential within their desired outcomes.
  • Maintains effective communications with clients, primary care physicians, substance abuse and mental healthcare providers, family, collateral resources and other Agency staff on behalf of clients.
  • Establishes and maintains appropriate and timely clinical case records, including, but not limited to, treatment plans, progress notes and treatment plan reviews, documenting appropriate therapeutic intervention with each client to the Agency, funding, licensing and billing authorities.
  • Works as part of the CORE team; attends and participates in team meetings to provide input/feedback around psychosocial conditions/comorbidities to review client status, up- date plans and goals, review outcomes to further program goals.
  • Acts as a resource/consultant to all team members on psychosocial and/or substance abuse issues and resources.
  • Provides telephonic as well as face to face outreach, engagement and service planning in the field.
  • Assist the program to provide crisis intervention and follow-up on an as-needed basis 24 hours a day, 7 days a week.
  • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
  • Complete QI Training during onboarding and participate in QI refresher training as needed.
  • Identify and implement quality improvement trainings and activities as part of professional development goals.
  • Encourage quality improvement processes and provide consistent resources for QI and professional development.

EDUCATION AND EXPERIENCE:

  • Professional licensure or permit from New York State Education Department is required for this position, i.e. LMHC, LMSW, Creative Arts therapist, Marriage & Family therapist
  • At least two (2) years' experience providing clinical services to individuals with mental health and/or substance abuse disorders.
  • Working knowledge of computer software and electronic health record
  • Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.
  • Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental treatment
  • Knowledge of Medicaid, Social Security and other entitlements
  • Excellent interpersonal skills
  • You must have the ability and willingness to regularly travel, in some instances with clients in Agency vehicles, to many locations using various modes of reliable and safe transportation. If working in Nassau or Suffolk Counties, a valid Tristate (New York, New Jersey or Connecticut) driver's license is required; and you must have a current and valid driver's license on file with CNGCS' Human Resources and be designated and re- main in good standing as a CNGCS authorized driver.
