Job Description
SUMMARY: Plans, organizes, and implements Behavioral Health Consultant role in the Primary Care Health Home.
BASIC FUNCTIONS OF POSITION:
- Work with Primary Care team to treat and manage patients with chronic health problems
- Assist in the detection of “at risk” patients and treating mental disorders and psychosocial problems
- Educate Primary Care Providers and staff on the Behavioral Health Consultant role, behavioral health topics and treatment
- Monitor behavioral health integration efforts through education, evaluation and suggestions to administrative team
- Conduct brief functional assessments and evidenced-based interventions to enhance a patient’s ability to achieve goals of the medical treatment plan. Meet with patient and Primary Care team to develop collaborative approaches to care and evaluate care plans
- Provide quality counseling and support to patients and families experiencing a mental health crisis or crisis involving injury and/or death
- As directed, conduct psychotherapy sessions with the use of evidenced based treatment modalities in accordance with state/regulatory standards
- Enhances professional growth and development through participation in education programs, current literature, in service meetings, and workshops
- Document in the electronic medical record
- Attend meetings and performs other related duties as assigned or requested
- Actively support and promote adherence to the policies and procedures of Missouri Delta Medical Center and Primary Care clinics
QUALIFICATIONS:
EDUCATION: Bachelor and Master’s degree from an accredited school required.
LICENSE: MO licensure as Provisional Licensed Professional Counselor(PLPC), Licensed Professional Counselor(LPC), Provisional Licensed Marriage and Family Therapist(PLMFT), Licensed Marriage and Family Therapist(LMFT), Licensed Master Social Worker(LMSW), Licensed Clinical Social Worker(LCSW), or equivalent level, OR licensed out of state and eligible for MO licensure.
EXPERIENCE: Counseling/Social work experience required.
SPECIAL:
PATIENT CONTACT: Must have the knowledge and skills necessary to provide care appropriate to patients and individuals of all ages and varied cultural and ethnic backgrounds on a daily basis.
PHYSICAL:
STANDING: Moderate daily.
WALKING: Multiple times daily in all areas of the hospital and weekly about the community.
LIFTING: Not required.
VISUAL: Multiple times daily to read charts and documents, assist visually impaired individuals and research resource materials.
HEARING: Multiple times daily to speak with patients and professionals, in person and by phone, and to assist hearing impaired individuals.
SPEAKING: Required at all times by phone and in person to communicate with professionals and patients who may be hearing, sight, and speech impaired.
