Responsibilities:

The Licensed Practical Nurse provides intermittent skilled nursing services to patients in the home setting according to the patient’s plan of care. As a Licensed Practical Nurse you will:

  • Perform agreed-upon tasks under the patient’s plan of care
  • Report significant findings to patient's physician and RN as they occur
  • Schedule a Case Conference/IDG with assigned RN to review patient's needs, problems, level of care and any changes in plan of care, including hospice admission with patient consent
  • Participate in Quality Assessment and Performance Improvement (QAPI) program
  • Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Qualifications:
  • Currently licensed as a licensed practical nurse by the State Board of Nursing
  • Must maintain a valid driver’s license, reliable transportation, and good driving record
  • Ability to work in a field setting and exhibited ability to make sound nursing judgment
