Home Care Case Manager

• $20-$25/hour > Weekly Pay
• Reports directly to CEO. You will have a leadership role and help develop our company
• Help build and manage your own personal care team
• Part-time with an option to move to Full-time as we grow

Interim Healthcare of Denver is seeking a proactive and dedicated individual to join our team as a part-time Home Care Case Manager. This position offers the unique opportunity to not only provide direct care to our patients but also to step into a leadership role as we grow and expand.

You will play a pivotal role in our ongoing success and will be responsible for providing high-quality care to clients while also growing, supervising, and mentoring your team. As your business grows, you will transition into a full-time leadership role, overseeing the day-to-day operations, managing staff, and driving the strategic direction of the company.

As a Home Care Case Manager, here’s a big-picture view of what you’ll do:

Supervision and Training:

• Lead and supervise your team of caregivers, providing guidance, support, and mentorship.
• Conduct training sessions and orientation for your team.
• Assist in screening, hiring, and retaining employees to meet the needs of the clients served.
• Perform regular performance evaluations and provide constructive feedback to staff.
• Oversee scheduling, staffing, and resource allocation to optimize efficiency and client satisfaction.

Patient Care:

• Provide compassionate and personalized care to clients in their homes, including assistance with activities of daily living and companionship.
• Conduct assessments and develop care plans tailored to each client's unique needs.
• Monitor and evaluate client progress, making adjustments to care plans as necessary.
• Build strong relationships with clients and their families, addressing any concerns or issues promptly and effectively.

A few must-haves for the Home Care Case Manager:

• High school diploma/GED. Be at least eighteen (18) years of age
• Minimum of 3 years of experience in personal care services, with a strong background in caregiving and patient support.
• Demonstrated leadership abilities, with experience in supervising and mentoring staff.
• Excellent communication and interpersonal skills, with the ability to build rapport with clients, families, and team members.
• Ability to pass a skills competency exam, background check, and drug screen.
