Summary

The Palliative Care Nurse Case Manager is responsible for managing a caseload of patients starting at initial consultation to Palliative Care and continuing through the lifespan or time of involvement with Palliative Care. Facilitates coordination of care by all members of the health care team to achieve appropriate outcomes.

Duties and Responsibilities

1. Assesses and procures community resources for families, while assessing for changing patient needs throughout the course of care with appropriate referrals for consultation, as needed.

2. Assists with development and implementation of materials necessary to facilitate patient care, patient/family/caregiver education and staff education related to care coordination throughout family healthcare experience.

3. Demonstrates leadership and interpersonal relationship skills which enhance communication and promote conflict resolution. Communicates effectively with patients/families, inter- and intradisciplinary teams, and outside resources.

4. Demonstrates knowledge and skills necessary to provide care for the physical, psychosocial, developmental, educational, and safety needs of patients served regardless of age. This includes the ability to perform independent home or outpatient visits to assess, address and/or triage palliative care needs appropriately.

5. Utilizes appropriate resources and coordinates health care team efforts to promote the achievement of positive family outcomes in a timely manner.

6. Facilitates, negotiates, and procures services and resources needed by patients to achieve desired outcomes. Often involved in complicated discharge planning, for both hospice and medically complex patients.

7. Communicates with various payors and ensures reimbursement for ACH services by facilitating proper prior authorization.

8. Works with families to help them gain the knowledge they need in preparation for the possible worsening or death of their child.

9. Participates in 24-hour psychosocial call with responsibility for pediatric palliative care patients. Psychosocial call is covered by various members of the team including chaplain, social workers, nurse case managers, and bereavement coordinator.

10. Provides palliative care focused education and clinical expertise and guidance for a wide variety of learners, including community agencies (e.g. hospice), nursing peers, undergraduate and graduate nursing students, pediatric palliative care fellows, medical students and pediatric residents, and social work interns.

11. Collaborates with research team to provide information as related to clinical practice and engages in quality improvement in patient care services and department functions.

12. Participates in orientation, training, ongoing mentoring, and evaluation of pediatric palliative care physician fellows as well as new hires to palliative care team, as appropriate.

13. Other duties required.

Technical Expertise

1. Experience in case management is preferred.

2. Experience in healthcare is preferred.

3. Must possess excellent oral and written skills.

4. Ability to move freely about the hospital and read/interpret written and graphical data is required.

5. Proficiency in MS Office (Outlook, Excel, Word) or similar software is required. Epic software or similar EMR software is preferred.

Education and Experience

1. Education: BSN required; MSN preferred.

2. Licensure: Licensed and currently registered as a professional nurse in the state of Ohio.

3. Certification: CCM or CCTM certification is preferred; CHPPN certification required within 3 years of date of hire.

4. Years of relevant experience: 3 to 5 years

5. Years of supervisory experience: None

6. Current healthcare provider BLS training from American Heart Association is required.

7. Ability to deal with confidential information and/or issues using discretion and judgement.

Working conditions

· While performing the duties of this job, the employee is regularly required to lift, walk, stand, sit, bend, reach with hands and arms, climb, push/pull, use hands, and see, hear and speak.

· While performing the duties of this job, the employee is in the standing position the majority of the time, except when documenting patient care.

· The employee must occasionally lift and/or move up to 50 pounds.

· The noise level in the work environment is usually quiet to moderate.This position works in an environment in which there is potential exposure to items that may present health problems. In order to mitigate this risk, the employee must wear appropriate protective equipment, demonstrate awareness of the environment, obtain knowledge of risk factors, and adhere to safety guidelines
