This is a FT position Mon-Fri 8:00am-5:00pm 

The successful Chronic Care Manager:

Graduate of an accredited School of Nursing

Minimum of two-years experience working in healthcare industry

Must be empathetic, and possess great communication skills

Essential Functions:

• Perform chronic care management services consistent with each assigned patient's orders and/or established care plan
• Serve as patient advocate by working with each assigned patient to establish and achieve personal goals for health improvement
• Serve as patient navigator by identifying and coordinating necessary healthcare and social support services for each assigned patient including, but not limited to, regular communications with caregivers and providers involved in patient care
