Position Summary:

Provides individualized assistance to patients, families, and caregivers to overcome health care system barriers and facilitates timely access to quality health and psychosocial care throughout the continuum of care for breast cancer patients. 

Reports to: Clinical Director

FLSA: Exempt

ESSENTIAL FUNCTIONS:

Provides education, support, and coordination to assist patients in securing appointments

Identifying, creating, and providing educational resources on breast health, breast cancer, and breast care to patients, their families, and caregivers as needed

Connecting patients and families to resources and support services

Promoting communication between patients and health care providers across the care continuum

Facilitating shared decision-making to ensure patients are fully informed of risks and benefits of treatment options, considering patient’s individual values and preferences are integrated into treatment decisions

Designs, evaluates, and recommends program and policy development for the care of breast cancer patients

Serves as a quality lead for the Breast Program Leadership Committee to maintain the National Accreditation Program for Breast Centers

Serves as a lead for the Breast Cancer Patient Support Group

Serves as an active member of Cancer Committee and Tumor Board


Minimum Qualifications:

Minimum Education
Associate's degree in Nursing
Bachelor's degree in Nursing (preferred)
  
Required Skills
Requires critical thinking skills, ability to prioritize and reprioritize quickly, strong leadership skills, and the ability to work autonomously. Ability to work collaboratively with both internal and external entities and have strong verbal and written communication skills. Must have basic computer skills and have strong organizational skills.
  
Required Skills
Licenses/ Certifications:
Registered Nurse (RN) in the State of Michigan
Navigation/ Oncology Certification within 1 year of taking the position as outlined in the National Accreditation Program for Breast Center requirements
  
Minimum Work Experience
5 years nursing experience required
5 years oncology nursing experience (Preferred)
