Job Description:

We are seeking a dedicated and compassionate Long-Term Care Pharmacist to join our team. As a Long-Term Care Pharmacist, you will play a vital role in ensuring the safe and effective use of medications for our long-term care residents. You will work closely with healthcare providers, nurses, and other members of the interdisciplinary team to optimize medication therapy and enhance patient outcomes.

Responsibilities:

  • Review medication orders and ensure accuracy and appropriateness of therapy.
  • Collaborate with healthcare providers to develop individualized medication regimens for residents.
  • Monitor medication therapy for efficacy, safety, and adherence to treatment guidelines.
  • Provide drug information and education to healthcare staff, residents, and family members.
  • Participate in medication reconciliation processes during transitions of care.
  • Oversee medication storage, distribution, and inventory management.
  • Ensure compliance with state and federal regulations governing long-term care pharmacy practice.
  • Maintain accurate and complete documentation of pharmaceutical care activities.

Qualifications:

  • Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program.
  • Active pharmacy license in the state of [Insert State].
  • Completion of a pharmacy residency program or equivalent experience preferred.
  • Previous experience in long-term care pharmacy practice is highly desirable.
