Clinical Coordinator (LBSW, LGSW) - Full-time / Part-time

Provides case management and counseling services to a client caseload assigned by the program manager.

Assess referred clients and leads development of service plan in conjunction with treatment team and funding sources in accordance with corporate

Creates, coordinates, and evaluates service plans through partnership with stakeholders based on assessed clients needs

Provides pre-service introduction and in-service continuing education to mentors

Makes mentor and family home visits and conducts mentor group meetings

Maintains current and complete client and reporting records in accordance with corporate and regulatory standards

Assists with all legal, licensing, and certification activities as needed

Assists with emergency coverage and crisis intervention support as needed

WORK SETTING

They will have a case load of 8-10 cases assigned and will be responsible for creating their schedule to see all their cases twice a week.

They will be working with children from infant to 21 years of age and their foster families and parents.

They will be required to travel across the Eastern shore to their homes so up to 3 hours and will be reimbursed for mileage

QUALIFICATIONS

Bachelor's or Masters Degree is Social Work: LBSW, LGSW, or LCSW

Two years of experience in the provision of mental health services with emotionally disturbed clients

Experience with children and families preferred

Active Driver's license, car registration, and auto-insurance required
